The California Health Advocates (CHA) Senior Medicare Patrol (SMP) and California Hospice and Palliative Care Association (CHAPCA) work with community organizations to inform this project.

For Hispanic/Latino community outreach, the strategic partner was Inland Caregiver Resource Center (ICRC).

For Chinese community outreach, the strategic partner was Chinese American Coalition for Compassionate Care (CACCC).
